Clarifications
INDOT projects are not eligible for the APAI 2011 Pavement Awards Program
Instructions
· Submit the APAI Quality Pavement Awards form with the $50.00 entry fee.
· Return completed form to APAI by fax 317-632-2445 or via Email to Charlie Holland at cholland@asphaltindiana.org .
· Form must be in the hands of APAI by November 3, 2011
· Any questions please contact Charlie Holland 812-619-1222
· Form is in word 93 you can download and type in the info from your computer.
· Include eight pictures total
o 4 each digital photos (preferred) completed project
o 4 each of Active construction photos ( if available)
